LiveWire COV motorcycle loan release form

LiveWire EV, LLC (“LiveWire”) insures the motorcycle on loan. To ensure against any unnecessary damage or loss of the motorcycle LiveWire imposes the following conditions of loan. Failure to comply with any of the conditions invalidates the insurance.

The motorcycle you have been allocated is loaned to you under the following conditions:

• You will guarantee its safe return, without damage and without delay.
• In the event of damage to the motorcycle resulting in an insurance claim, you agree to pay the minimum insurance excess charge of €750,- directly to LiveWire.
• You agree to take all reasonable care of the motorcycle while it is in your care and to ensure that wear and tear is kept to a minimum.
• If any delay in the return of the COV results in the next booking being postponed, it will result in you not being able to loan a COV in the future.
• Under no circumstances do you take the bike to a third-party supplier, technician or service department outside the authorised LiveWire network, for any reason whatsoever.
• The motorcycle remains the property of LiveWire UK Ltd., and you will not lend, mortgage, charge or otherwise part possession of the motorcycle during the period of the loan.

Although LiveWire insures the motorcycle on loan, cover depends on you being able to make the declarations listed below. If you are unable to give any these declarations, you are required to contact LiveWire so that insurance coverage can be confirmed or arranged. I hereby confirm:

• I am over 25 years of age.
• I have a held a full licence to ride a motorcycle for more than 12 months with no current UT, CD, DD or DR coded penalties.
• I have not been disqualified for a period exceeding 6 months in the last 3 years.
• I have not been disqualified for a period exceeding 1 month in the last year.
• I have not been involved in more than 3 accidents in the last 3 years.
• I have not had more than 2 theft claims in the last 3 years.
• I am experienced in and familiar with the operation of motorcycles and fully understand the risks and dangers inherent in motorcycling.
• I am voluntarily participating in the ride and I expressly agree to assume the entire risk of any accidents or personal injury, including death, which I might suffer as a result of this ride and hereby waive and release LiveWire EV, LLC and its subsidiaries, affiliates, parent and holding companies (collectively the "Company") from all and any claims, demands, rights, causes of action or other liability whatsoever arising from or in connection with my use of a COV. Nothing in this Release Form shall, however, operate to release the Company from any liability for death or personal injury, which results from any negligence of the Company.
• I agree to return the motorcycle on or before the date specified below to the specified location.
• I undertake to ride in a safe and responsible manner, to wear an approved motorcycle helmet, eye protection, protective clothing, ankle boots and gloves while riding.
• I agree to take all reasonable care of the motorcycle while it is in my care and to ensure that wear and tear is kept to a minimum.
• I agree that I shall not permit any other person to operate the COV or ride as a passenger unless they have first completed any consent and release form required by the Company.
• I agree to ensure that the motorcycle is locked and / or garaged while in my care.
• I understand that any traffic offences or parking fines accrued while the motorcycle is on loan to me will be my responsibility to discharge.
• I will notify the COV Fleet Manager of any problems or mechanical concerns that have occurred during the loan period.
• I have read, and agree to abide by, the above terms and conditions.
